Our hospice care is not confined within our building; instead, we strive to reach out to our community, offering compassionate support and care to those in need. This has been made possible by extending our volunteer network, growing connections and forging closer ties with partners.

Expanding Community Support

Under our strategic initiative “Adapting to a Changing World,” we have been focused on expanding our community support. The aim is to bring hospice care as close as possible to people’s own homes in line with national strategies. This shift in focus has yielded remarkable results, with a 48% increase over the last 4 years of our current strategy in the number of people supported across all clinical services. Our “Hospice at Home” service has played a vital role in achieving this, allowing patients to receive the same range of support in the comfort of their homes as they would expect as an inpatient.

Strengthening Patient and Family Support

Our patient and family support services have seen a surge in demand, especially due to the impact of the COVID-19 pandemic. To cater to this, we have extended our virtual and face-to-face support, ensuring accessibility and choice for those seeking our services. We have also focused on community collaboration projects, providing outreach support to those who would otherwise be unable to access our services.

Compassion in the community

Our Compassionate Neighbours initiative has seen our incredible volunteers generously share their time and kindness with local individuals who may be experiencing social isolation, loneliness, or facing life-limiting conditions. We have one simple goal: to unite people based on their shared hobbies, interests, and geographic proximity, fostering authentic and caring friendships that create a sense of belonging and help build stronger, tightly-knit communities. In the past 12 months, our compassionate team has made 1,673 connections within our community — an 80% increase compared to last year’s interactions.
