3. Hospice at Home

Our community services work collaboratively under the service title ‘Hospice at Home’. Community care is an integral component of our service, allowing us to bring the skills, ethos and practical care associated with the hospice movement into home environments.

We’re seeing increasing numbers of patients opting to receive care at home or in their usual place of residence. Our team supports people across Edinburgh and the Lothians to stay where they feel most comfortable and safe, surrounded by memories created over the years and the people they love.

In 2023/24 our Hospice at Home Teams supported 395 people to die at home and 132 people to die in a hospital setting

The Care at Home Team provides personal care and support for people who have received a life-limiting diagnosis or, are at the end-of-life. Last year, the team supported 280 patients and carried out 2,614 visits.

Enabling people with a life-limiting illness to remain in familiar and comforting surroundings is the main aim of the Community Palliative Care Team. They’ve supported 870 patients over the past year to manage complex physical and psychological needs, discuss anticipatory care planning and co-ordinate end-of-life care.

The Virtual Ward provides medical and nursing support in people’s own homes, mirroring the same level of care patients would receive in the Hospice itself. Over a 12-month period the team supported 160 patients. Working in partnership with District Nursing and Marie Curie Rapid Response teams, they are successfully delivering enhanced care and support in our local community.
