We understand it can sometimes feel confusing or overwhelming to know where to start accessing services offered by the Hospice.
Our priority is to understand what is happening for you in that moment, address any immediate issues and get you to the most appropriate support as quickly as possible.
We receive and triage all referrals and undertake first assessments with patients and supporters via telephone. We arrange all inpatient admissions and manage the inpatient waiting list to ensure that we prioritise the people with most need.
We work with District Nursing and GP colleagues to provide them with complex symptom advice and liaise with wider health and social care teams.
Our team is also able to respond to requests from patients and supporters for general advice and information as well as clinical services information.
