If it’s the right thing for you, our Community Palliative Care Team will help you manage your symptoms and provide you with access to the services you need to live well in your own home. Our multidisciplinary team of nurses, physiotherapists and occupational therapists provide outpatient appointments, day therapies and home visits. Working closely with your own GP, they ensure you and your family have all the support you need.
Our Day Therapies service is a rehabilitative service available to anyone living at home at any stage of an incurable illness. We offer a range of classes designed to help you live as independently as possible. These are safe spaces, designed to help you reconnect with the things that matter to you, explore different ways of going about your day-to-day life, meet other people in similar situations to you and rediscover hobbies you might have forgotten about.

Drop in session
Most of our services are by referral, but we run drop-in sessions that you don’t need an appointment for. These sessions are designed to enable you to find out about our services and meet other people in similar situations.
